Request for Motor Vehicle Administration Records - English Version (Form #DR-057) ( Spanish Version​) is an application form used to request motor vehicle records. This form can be filled out and brought in person to your local MVA office or mailed to the Maryland Motor Vehicle Administration, 6601 Ritchie Highway NE, Glen Burnie, MD 21062. Please send requests for vehicle records to room 206. Please send requests for driving records to room 145. The remittance fee must be in the form of a check or money order made payable to the Motor Vehicle Administration (MVA). Checks must have an imprinted name and address. Please include your driver license number and your home or work phone number on your check.

​Surety Bond of Information Requestor Driver/Vehicle Records (Form #DR-082) is an application used by insurance companies to grant bonds for online Record Services for Direct Access Records Systems or License Monitoring System. Must be mailed to the Motor Vehicle Administration, 6601 Ritchie Hwy., N.E., Glen Burnie, MD 21062, Room 145 in conjunction with other forms to be approved by the Administration.
